Investigation Summary

Investigation Nr: 202312799
Event: 09/20/2000
Employee Injured When Hand Caught In Pulley

At approximately 6:00 p.m. Employee #1, an assembler, was performing a "final check" on one of his company's product known as a "mechanical synchronizer. This is a final systemic check before sale. At this time Employee #1 was readjusting the machine's shaking motion from vertical to circular. To perform the procedure correctly, the machine must be partially functional and have a 400-pound test weight placed on its scale. In counterbalance to the imposed weight the internal mechanism is under quantitative tension. As Employee #1 was removing and realigning two internal bolts necessary to change the position he did not observe that the "keyway" was in the "up" (unsafe position) as opposed to "down" (safe position). At this point it is possible for the eccentric (a large bushing) to turn at least a one half rotation and to release stored quantitative energy imposed by the 400-pound weight. Employee #1's left hand was on the pulley belt when the unexpected rotation occurred. Employee #1's fingers were immediately caught and crushed in the unguarded "pinch point" belt and pulley system. Employee #1 was treated for his injuries but was not hospitalized.
