Employee fingers amputated on a table saw

Employee #1 was assembling parts of a cabinet and noticed one of the parts needed another cut to fit. His coworker had been cutting the pieces but had turned around to do something else. Employee #1 walked over to the operating side of the table saw and noticed that the blade guard was lying on the floor. He did not attach the guard and proceeded to make the cut. He completed the cut and reached across and behind the blade to remove the scrap piece. He dragged the scrap toward himself and it caught on the blade and pulled his hand into the rotating blade. Several fingers were amputated but re-attached at the hospital.
