Investigation Summary
Employee #1 was attempting to solve the problem of the warped edge of the web, corrugated paper, creating incorrect lengths when fed into the score blades and then through the knives. He placed his left hand on the warped 49-in.-wide web as it fed from left to right before it reached the score blades. He turned to his left and it is suspected that he either placed or brushed his right gloved hand on the web. The glove had rubber dots on the palm and it was grabbed by the web which pulled his right arm into the rolls that hold the score blades in place. The rolls were about 4 inches apart and have a key-way that runs the length of the roll running at 680 fpm. Employee #1's arm went into the rolls up to his arm pit and the key-way ripped open the area under his arm. His arm was in the machine for less than a minute when someone heard his call and hit the emergency stop button. He was able to extract himself from the rolls and he was hospitalized with cuts and lacerations.
