Investigation Summary
On September 18, 2008, Employee #1 was working at wine storage warehouse providing shipping and storing services for importers, wineries, wholesalers, retailers, private collectors and auction houses. He used a Raymond Order Picker lift truck, Model Number 540- OPC30TT, with a pallet attached to collect 15 cases from a rack located 12 ft high. He had 13 of the 15 cases loaded and positioned himself with his left knee on a case and his right foot on the front end of the pallet. The pallet end broke and Employee #1 fell to the concrete floor and was hospitalized with a concussion and rib injuries.
