Investigation Summary
At approximately 2:35 p.m. on July 28, 2008, Employee #1 and a coworker, painters, were painting a three story building. Employee #1 had been straining and pouring paint into a sprayer reservoir for the coworker. The coworker ran out of paint and called to Employee #1 but got no response. He found Employee #1 unresponsive, on the ground. He fell approximately 31 ft off of the scaffold and died.
