Investigation Summary
At 3:00 p.m. on September 13, 2007, Employee #1, who worked for North Bay Sunspace, Inc., was installing roof panels for a four-season sunroom at a private residence. One of the panels was not straight and he was trying to bend it when he lost his balance and fell 8 ft to the floor. He sustained a fractured hip and head injuries, for which he was hospitalized.
