Employee's Leg Is Fractured When Pinned Under Falling Load

At approximately 1:22 p.m. on August 24, 2006, Employee #1 and coworkers were in the process of pouring concrete around the top edge of a 2 ft by 5 ft by 8-in., 1,000 pound precast concrete catch basin lid. They were using a backhoe for lifting the concrete member and a steel bar for adjusting the concrete members. The lid was not secured, shifted, and fell while Employee #1 was standing on the top edge. He was pinned by the lid on the north wall of the 10 ft deep catch basin. Employee #1 received a trauma to his lower left leg and was hospitalized.
