Employee Is Injured in Fall from Lift Basket

At approximately 10:09 a.m. on February 16, 2006, Employee #1 was standing on top of a transport trailer placing tarp over the open trailer of recycled materials. After Employee #1 finished placing the tarp, he asked a coworker, operator of the industrial truck, Toyota, Model Number 5FD25, Serial Number 45603, to lower him down to the ground. While Employee #1 was being lowered, at approximately 8 ft, the lift malfunctioned and fell uncontrollably to the ground. During the fall, Employee #1 struck the fork guard and the pavement. Employee #1 was hospitalized and treated for a fracture of the right transverse processes of lumbar vertebrae L1 through L3. Employee #1's Employer recycled post consumer waste and rented large debris containers. Industrial trucks were an integral part of transportation of equipment and materials.
