Investigation Summary
At approximately 9:30 a.m. on March 24, 2007, Employee #1 was working in the furniture workshop. Employee #1 was tailing-off on the Homag (Type KL/78/QA-1, Serial Number 0-200-08-3931, 480 Volt) edge bander. After finishing the pallet of table tops Employee #1 helped to clean up the area. In order to clean the edge-bander's dust collection duct Employee #1 removed the duct and reached into the shaper's exhaust port, to remove debris. Four fingers of Employee #1's left hand became entangled in the shaper blade, resulting in the amputation of one finger and lacerations to the other fingers. Employee #1 was hospitalized.
