Investigation Summary
On October 26, 2004, an employee, a woodshop supervisor, as next to the CNC machine Anderson Industrial Corporation, Exxact Model Number 51, Serial Number 01-88007 multi- head router with 5x10' table. The employee observed a piece of scrap 0.625-in.-thick plywood bouncing around loose on the Exxact 51 CNC router and reached to remove it to prevent it from jamming. As he reached in and started to lift it, the machine's table moved back to go to its next work position. The employee's little and ring fingers on his right hand were squeezed between the plywood, the table, and the machine frame, crushing and fracturing them at the bottom knuckles. The employee was hospitalized, and the fingers were repaired at the hospital emergency room. On November 18, 2004, the two fingers were surgically amputated due to complications.
