Concrete Form Struck Employee Knocking Him Over Wall

On July 27, 1999, Employee #1 was working with a crew of men removing forms from poured concrete columns. The forms were suspended by ropes. As one of the forms came free it swung and struck Employee #1, knocking him over the guardrail to the floor below. Employee #1 was hospitalized for bruises, contusions and abrasions.
