Investigation Summary
An employee was operating a five-cubic-yard concrete mixer when he dropped one of his gloves into the mixing bowl. While the mixer was operating, he attempted to retrieve the glove by lifting the grate guard and reaching into the mixing bowl. When he did, his right had was caught by the mixing blades and amputated. He was transported to the hospital where his hand was successfully reattached.
