Investigation Summary
Employee #1 was involved in the construction of an eight-story cast-in-place concrete parking structure. The construction of this structure consisted of using "tables", made of braced thick plywood, weighing about 800 lbs. that sit on either side of steel beams. A table became hung-up and Employee #1 attempted to lift the table with his hand. The table shifted and his right middle was caught between the table top and the metal beam. His finger was amputated at the tip.
