Investigation Summary

Investigation Nr: 201122405
Event: 05/05/2000
Employee's Fingertip Is Amputated When Crushed by Board

An employee was on the 50 foot high platform of an aerial lift, stripping the lumber formwork from under a concrete bridgeway under construction. He was throwing away a 20 foot long piece of four by four lumber when it became caught on an angle bracket. This caused the far end of the board to tilt downward and the end the employee was holding to become jammed against the concrete surface under the bridgeway. The employee's right ring finger was crushed between the four-by-four and the concrete, amputating the tip.
