Employee%92s fingers amputated in forks of a high lift truck

At approximately 2:30 p.m. on October 6, 1999, Employee #1 was preparing to have himself lifted on a pallet resting on the forks of a high-lift truck to disconnect slings from a piece of steel. With his hand on the back frame of the forks, the mechanism was put into motion when the Project Manager called out a warning. The forks moved and his fingers were crushed, resulting in one of them being amputated. Employee #1 was hospitalized.
