Employee Injured When Forklift Fork Falls On His Hand

At approximately 7:30 a.m. on June 1, 1999, Employee #1 was using a forklift to move construction materials on a job site. Employee #1 needed to move the adjustable forks to a wider position. Employee #1 did this by pulling on one fork by hand. The fork came off the end of the slider bar and fell onto his hand, breaking Employee #1's fingers. The slider had been broken a few days previously and not been repaired. Employee #1 was hospitalized.
