Investigation Summary
At approximately 11:15 a.m. on July 15, 1999, Employee #1 was training another worker in the operating procedures of a Pot Spacing Machine. A flower pot had fallen over and Employee #1 switched off the machine and stepped over a low guard to set the pot upright. The trainee switched the machine back on, having misheard the instruction Employee #1 had given and the machine moved. The rotating crank arm caught Employee #1's leg and pierced it. He was hospitalized.
