EMPLOYEE FALLS WHILE REPAIRING RUBBER SKIRTING

Employee #1, an operating engineer, was on top of a conveyor belt of a concrete aggregate batch plant to repair a torn rubber skirting. After the operator at the control room started the belt, Employee #1 fell 10-12 ft to the concrete floor below. Although he followed the company's lockout procedures, he locked the wrong control electrical box. Employee #1 was hospitalized with fractures. Earlier, when the injured requested permission to repair the belt, he was told by the operator not to take the conveyor out due to high production demands.
