Investigation Summary
Employee #1 was on the work platform of a pump jack scaffold, installing siding on a single-family home when he suffered a seizure and passed out. He collapsed and fell approximately 8 ft 4 in. through the guardrail nailed to the outside of the uprights. Employee #1 ruptured his spleen as he struck the ground; he died as a result of his injuries. Employee #1 was working alone at the time of the accident. The employer was cited for a serious accident-related violation.
