Investigation Summary
Employee #1, who worked for a well drilling company, was removing the blowout protection from a drill rig at a steam field construction site. He was on a scaffold guiding a 30 ft tall irregular drill stack when the raised stack upset the plank on which he was standing, causing him to fall. Employee #1 sustained serious spinal damage and underwent surgery to fuse his vertebrae. He was an experienced well driller. This was a planned operation, and two supervisors were in the immediate area when the accident occurred.
