Investigation Summary
Employee #1, a 19-year-old worker at wood pallet manufacturing facility, was cutting 2 by 4 material with an under-hung Precision Products Company chop saw, model I8M, serial #18366. He was standing at the operator's position when he reached into the point of operation with his left hand to remove some scraps. He inadvertently hit the knee activation switch, amputating his left thumb. The top guard was set for 4 by 5 material, not 2 by 4 material. This allowed Employee #1's hand to access the unguarded knee switch, which he accidentally bumped, activating the saw.
