Investigation Summary
At approximately 9:15 p.m. on April 4, 1999, Employee #1 was working with the bread wrapper machine. Some plastic had become jammed at the blade, and he reached in to remove it. The machine actuated, amputating his middle finger at the first joint and the tip of his index finger. The guard at the blade's point was not effective. It had enough space to allow his hand in the blade's area without causing the machine to stop. The guard SHUT-OFF switch had a broken weld and was positioned in such a way that it did not provide adequate coverage. The company's lockout/tagout policy and procedures for clearing the plastic jam were not followed. The employer was cited for a violation of T8CCR 3314(a), concerning cleaning, repairing, servicing, and adjusting machinery and equipment.
