Employee Lacerates Feet in Auger

At approximately 6:50 a.m. on November 4, 2003, Employee #1, a cellar employee for a winery, was replacing an overhead drum onto its supporting rollers. Employee #1 was standing with his feet on the sloped side of the hopper above the auger. He was positioning the drum when he accidently closed the probe contacts on the hopper. The Vaslin-Buckner Must Pump Auger PM60 (Series Number 019300, Model Number 374) which was not locked out, was activated. The hopper top opening was 43-inches by 43-inches and the auger in the bottom of the hopper was approximately 10 inches in diameter. Employee #1 sustained lacerations to his feet and was hospitalized.
