Employee amputates finger with table saw

At approximately 6:02 p.m. on November 17, 1999, Employee #1 was operating a table saw to cut a 12 in. by 12 in. block of polystyrene foam. As he was cutting the foam, the blade went through a void accelerating the foam and his left hand into the blade. The blade amputated his left middle distal phalanx at the first joint. He was hospitalized to receive surgical treatment for the reattachment of his severed finger. At the time of the injury, there was no guard over the blade of the table saw. The employee stated that there has not been a guard on the table saw for some time.
