Employee falls and dislocates his back

At approximately 2:55 p.m. on September 17, 1999, Employee #1, a foreman, was working with three other workers attaching a raked wooden wall to a stationary blocked wall approximately 12 ft 7 inches high. A Gradall Forklift, Model Number 534C, Serial Number 0288876 was moved into position with the raked wall suspended on a sling from a forklift blade. The tension on the sling slipped causing the wall to wobble which caused Employee #1 to loose his balance and fall approximately 13 ft. He dislocated his back and was hospitalized.
