Employee gets struck by forklift attachment

At approximately 11:45 AM, on July 16, 2008, Employee #1 was supervising the unloading of building stones from a container truck. The stones were bundled and mounted on pallets. The pallets were pulled out to the open end of the container with a CAT front-end loader using a 0.375 inches 20 foot metal chain with one end attached to the tow pin of the loader and other end attached to a pallet puller (i.e. scissor clamp). The clamp was then attached to the middle rib of the pallet and pulled. The injury occurred when the clamp broke while a pallet of stones (weighing approximately 6400 pounds) was being pulled. The strain on the chain caused the clamp to be violently pulled backwards. It hit employee #1 who was then standing midway between the loader and the container on the left side of the loader facing the container (about 10 feet from the open end of the container). Employee #1 was hit on the right arm and side of the body and sustained serious injuries.
