Employee killed when neck is broken in concrete cuber

At approximately 1:05 p.m. on May 13, 1997, Employee #1 was performing his routine duties as operator of a concrete block cuber when he apparently became caught by the sweeping steel push beam. When the machine cycled, Employee #1's neck was fractured between the push beam and a vertical steel strut. The cuber machine did not have a guard. Employee #1 was working alone at the time of the accident.
