Employee killed in 39 foot fall from steel beam

At approximately 2:15 p.m. on December 8, 1994, Employee #1 and a coworker were working on a metal roof, placing metal decking on a steel joist. While standing on a steel beam, Employee #1 reached down and picked up a sheet of 3 foot by 27 foot decking. He dropped the sheet, lost his balance, and fell backward through the steel joist. Employee #1 fell 39 feet to the concrete floor. He died from severe head and body injuries.
