Employee dies of head injuries in fall from machine

At approximately 6:30 a.m. on January 12, 1998, Employee #1 and a coworker were performing maintenance on a piece of equipment. Employee #1 fell approximately 15 ft from his work station inside the machine to the ground. He died of head injuries sustained in the fall.
