Employee dies in fall through roof opening

At 10:02 a.m. on August 27, 1998, Employee #1 and a coworker were on the flat, rubber membrane roof of a 100,100 square ft, 24 ft tall single-story structure. They were replacing temporary covers over the 2 in. by 4 in. by 14 in. metal frames set into the roof for the HVAC exhaust blowers. Employee #1 was kneeling on the west, elongated side of an opening, while his coworker was at the northeast corner. The two had set a 2 in. by 6 in. lumber frame in place and were preparing to secure 2 in. Carlisle foamcore panels on top of it. When the coworker turned back around, Employee #1 was mot there. A coworker inside the building saw Employee #1 fall 22 ft through the opening, landing head first on the concrete floor. He was flown by helicopter to Fairfax Shock Trauma Center, where he underwent surgery and was transferred to the ICU for possible recovery. Employee #1 died at 5:36 p.m. that same day of complications related to his severe head injuries.
