Employee killed in fall when rotten pole fails

Employee #1 was removing wires from a 50 foot high wooden pole. He cut a 1 1/2 inch diameter cable and then clipped the cable's restraining wire. The bottom section of the pole was rotten and the pole fell with the employee still attached to it. He suffered a concussion and was killed.
