Investigation Summary

Investigation Nr: 170387831
Event: 03/25/1997
Employee killed when pulled into auger

At approximately 1:15 p.m. on March 25, 1997, Employee #1 was assigned to watch the flow and to prevent clogs of aluminum pieces as they were moved by air current into the hopper of an Eddy shredder. Approximately 5 ft from where he was working, a 20 in. diameter horizontal auger was moving small amounts of dirt to a conveyor. The base of the screw was approximately 34 in. from the ground cover, and it had an approximately 28 in. long by 6 in. wide rotating shaft attached to the end bearing blocks. The shaft was partially guarded by a support beam. For some reason, Employee #1 left his work station and approached the shaft. His clothes apparently became caught on the protruding bolts, and he was pulled into the auger. Employee #1 was killed.
