Investigation Summary
Employee #1 was inserting a metal part into a 48-ton full-revolution mechanical power press. The part did not fit exactly, so she slid her hand under the guard to push the part down. As she did so, her foot struck the foot pedal and activated the press. All four fingers on her right hand were amputated to the first knuckle.
