Employee's thumb amputated in power press

At approximately 12:00 noon on or about January 28, 1998, Employee #1 was using a 200 ton Niagara part-revolution mechanical power press. He returned from lunch and looked at the control panel, thinking the press was on single stroke mode. He cycled the press while reaching into the point of operation for a part, amputating his left thumb. The press was in continuous stroke mode and re-cycled because the microswitch for the barrier guard had been tied open with a 25 in. long by 3/4 in. wide piece of flat bar stock so the press could run continuously with the barrier guard in the 'up' position.
