Investigation Summary
At approximately 1:15 p.m. on July 28, 1998, Employee #1 was operating a 15 ton Sheridan part-revolution press. He was inserting a puzzle into the press when it cycled, amputating the tips of three fingers on his left hand, and a fingertip and finger to the first knuckle on his right hand. The press was equipped with two-hand controls and a light curtain.
