Employee killed when head crushed in tire press

At approximately 7:15 a.m. on December 12, 1994, Employee #1 was inspecting a tire press for a malfunction. While he was inspecting, his head was in a pinch point formed by the whipple tree and the bottom of the press platform. No other employees were in the area when his head was crushed and he was killed. A guard had been removed for access. Energy sources were not locked out or blocked.
