Employee killed in fall with elevated work box

Employee #1 was installing exterior insulation on apartment buildings for a contractor. He was working from a three-sided wooden trash/debris box elevated by a Sky Trak forklift. A coworker on the roof of the building asked him to raise the box to the roof so he could ride down to the ground. Employee #1 motioned to the forklift operator to raise the lift. When the lift was near the peak of the roof, Employee #1 moved a little to the left, which caused the box to fall off the forklift. He dropped 20 ft and became pinned under the trash box. Employee #1 was killed.
