Employee killed by blade thrown from shaper

Employee #1 had installed a three-blade head on a vertical-spindle shaper. The blades were held in their slots by friction and pressure from the bolt and the spacer on the spindle. Employee #1 moved behind a protective barricade and started the shaper, which operated for approximately three minutes with no problem. Employee #1 then moved next to the shaper and leaned over to make a final adjustment. The blade came out of the shaper head and struck Employee #1, causing a massive, fatal head injury.
