Employee's fingers amputated in mechanical power press

At approximately 10:30 p.m. on March 13, 1995, Employee #1 was stamping metal parts on a mechanical power press activated by a foot pedal. A part was stuck in the die so he reached with his right hand to remove it. Employee #1 activated the foot pedal, causing the press to cycle and to amputate four fingers.
