Investigation Summary

Investigation Nr: 200070142
Event: 11/12/1996
Employee dies after suffering heart attack

Between 8:00 and 8:15 p.m. on November 12, 1996, Employee #1 was approached by a coworker who told him that she was going down to the vibrating room to work on a test, and asked that he check on her in an hour. At approximately 8:55 p.m., the coworker checked her watch and realized that Employee #1 would be coming down shortly. He failed to appear, and the coworker headed back upstairs at 9:15 p.m. Along the way she passed a security guard, who was making his routine rounds. Once back in the lab, the coworker found that Employee #1 was not at his desk and surmised that he must have gone to the restroom. She prepared to leave work, and left Employee #1 a note reminding him to remove some samples from a chamber. At approximately 9:24 p.m., she was locking her desk when the security guard came into the lab and said that he had found Employee #1 in the upstairs bathroom, unconscious. Emergency services was called, and the security guard performed CPR, but efforts to revive Employee #1 were unsuccessful. He died of an apparent heart attack.
