Employee killed in fall from stepladder

Employee #1 was on the fourth or fifth rung of a 10 ft fiberglass stepladder. He was using hand tools to remove wooden forms from a concrete wall when he fell from the ladder and was killed. Employee #1 was newly hired at the time of the accident.
