Employee killed when struck by flying pipe

Employee #1 was involved in the installation of an approximately 4 ft long pipe or riser on a 2 in. ball valve in preparation for blowing down a 6 in. gas line that was at 110 psi. When Employee #1 opened the ball valve to blow down the line, the riser separated from the valve and struck him on the back of the head. Efforts to resuscitate him failed, and he was pronounced dead at the scene by the County Coroner. Witnesses could not verify seeing Employee #1 use a pipe wrench to tighten the riser, nor could a coworker confirm that he had tightened it himself.
