Employee killed in fall from beam

Employee #1, a 56-year-old with prior steel erection experience, was standing on a 12 inch beam that was elevated 16 feet above the ground. When he unhooked two slings that had been used to raise a stack of purlins, he fell from the beam and landed on the cement. Employee #1 was killed.
