Employee falls off ladder on roof to the ground and is kille

On August 3, 2017 an employee was working alone and performing shingle repair on the upper 6:12 sloped and lower 4:12 sloped roofs of a building. The employee had used a two section 24' long metal Louisville extension ladder to access these roofs with the upper 12 foot section to access the lower roof from the ground and the lower 12 foot ladder section to access the upper roof from the lower roof. As the employee descended the ladder from the upper roof to the lower roof sections, the legs of the ladder slid out causing the employee to fall from the ladder onto the lower roof and then fall an additional 8 feet and 9 inches from the lower roof to the ground below. The employee died of spinal injuries caused by the fall.
