EMPLOYEE INJURED WHEN SCAFFOLD FELL WHILE CHIPPING SLAG
Employee #1 was working on a suspended scaffold chipping slag for a blast furnace reline. A large piece of slag broke the scaffold platform, causing Employee #1 to fall. No fall protection device was in use.
