Employee killed in paper machine

On August 12, 1995, Employee #1, of Simpson Paper Company, was the fourth hand assisting in changing the felt on a paper machine. The machine was running slowly. As the cut, loose end of the felt came off the press section, it hit an overhead light fixture and pulled out the plug. A coworker, the third hand, went up to a catwalk and asked Employee #1 to hand him the plug. As he got into position, he heard a thump and saw Employee #1 in the machine. He was crushed and killed. Apparently, Employee #1 stood on and fell off the standard barrier while attempting to plug in the light.
