Employee killed when crushed by loader/unloader arm

On June 13, 1995, Employee #1, of American Axle and Manufacturing, Inc., was working on the loader/unloader to a carrier-line presence-sensing limit switch without locking out the machine. He pushed the limit switch down and the loader/unloader arm dropped and struck him. Employee #1 died of a crushing injury to the head.
