Event: 12/29/1994
Employee crushed to death by falling pipe

On December 29, 1994, Employee #1 was flame cutting a section of steel pipe in preparation for shipment. The section was approximately 11 ft 4 in. in diameter and 11 ft 1 in. long. Employee #1 made his horizontal cuts and left his position behind the controls to hold the cut section until an excavator could pull it down. As Employee #1 walked on the outside of the pipe, the cut section fell onto him. He was taken to Mid-Michigan Regional Hospital, where he died on January 1, 1995.
