Investigation Summary
On February 22, 1995, Employee #1, of V & T Steel Fabricating was dumping metal into a dumpster with a small Hi-Lo. He attempted to pull metal from the dumpster with the forks extended all the way up. The Hi-Lo tipped over and the employee's head struck the parking lot's paved surface. He died. A citation was issued for a repeat serious violation of 4081-2154(1).
