Employee dies after falling from stepladder

On September 19, 1994, Employee #1, of Glen's Market, was restacking stock on an open-sided platform eight feet above the floor. She fell, striking her head on the floor, and died. The employee had gained access to the platform from an inadequate stepladder; she also had to step on the top step of the ladder to reach the platform. The employer received a serious citation.
